Privacy Policy

This policy explains how Luma Psychology handles personal and health information. It is written in plain language and may be updated from time to time.

What information is collected

When you send an appointment request through this website, the information you enter in the form is collected. This includes your name, date of birth, contact details, suburb and state, the concerns you would like support with, brief information about your current support and safety, your appointment and funding preferences, and any document you choose to upload.

If you become a client, further information is collected as part of the practice's formal intake and clinical record process. That may include your residential address, emergency contact, health and medication history, GP details and consent documents.

Why it is collected

The information in the appointment request form is used to review whether the service is likely to be a good fit for what you are looking for, to respond to your request, and to arrange care if you become a client. If you become a client, information is used to provide psychological services and to keep the clinical records required of a registered psychologist.

Please only provide the information needed for your enquiry.

Who can see it

Appointment requests are reviewed by Della Zhang. Information may also be handled by the service providers that host this website and the practice's client-management and email systems, in the course of providing those services.

Your information is not sold, and it is not disclosed to others for marketing. Health information may be disclosed to another health professional, such as your GP or referring doctor, where you have consented or where disclosure is required or permitted by law, such as where there is a serious risk to someone's safety.

Storage and retention

Information submitted through this website is stored in the practice's website database and file storage, and clinical records are kept in the practice's client-management system. Access is limited to Della Zhang and the service providers involved in operating those systems.

Health records are retained for the period required under Australian law and the relevant professional obligations, and are then securely destroyed or de-identified.

Access and correction

You can ask to see the personal information held about you, or ask for it to be corrected. Requests can be made by email and will be responded to in line with the Australian Privacy Principles. In limited circumstances access may need to be provided in another form, and reasons will be explained to you.

Questions or concerns

If you have a question or a concern about how your information has been handled, please contact Della at lumapsychology@hotmail.com. If you are not satisfied with the response, you can contact the Office of the Australian Information Commissioner.
